Notice required to cancel or reschedule, and fees for late cancellation or no-show:

- Office visits: ≥24 hours notice or $150 fee.

- Office-based procedures: ≥24 hours or $250.

- Outpatient surgery: ≥7 days or $300.

Fees are not covered by insurance (Medicare, Medicaid, private); patient responsible.

Outstanding balances (including these fees) must be paid; unpaid balances may block further non-emergency scheduling/services.

Late arrivals: Late arrivals wait until on-time patients are seen. >15 minutes late → physician approval to see or reschedule.

New patient booking fee: All new patients, whether insured or self-pay, pay a $200 booking fee to schedule their first visit. For insured new patients, the fee is refunded in full after they check in for their visit. For self-pay new patients, it counts toward the $400 new-patient visit.

The $200 booking fee is non-refundable if the patient does not show for the appointment.

Exceptions: documented medical emergencies / extraordinary situations — management discretion.
